On the wyndy Milla.. it is in the build que. Currently a THM stem and Enve aero 37/42cm handlebar. This will mean external cables from handlebars to frame although I am running Etap (think TCR or Aethos)

I could go internal cables with Deda alanera or Vision 5D ACR... I would look slick but be heavier and couldn't get as narrow..

As I want to use this bike to travel to Europe would I be mad to change to internal cables?

If I was buying a bike knowing I would be flying with it a lot I'd probably steer towards external rather than internal cables. If it was just a few trips (once or twice a year) then it wouldn't be an issue for me.

Go for FSA's SRS headset system so you can keep the THM stem while still maintaining some semblance of cable management. See Titici's implementation to get what I'm talking about.

In my opinion, there's nothing cheaper looking that seeing a mess of cables. Makes the bike look like a homemade bomb!

Is this the best option for this sort of set up? Anything lighter/more Gucci?

To my knowledge only Token's S-Box has similar functionality to FSA's SRS system. I'm not sure about lighter but it is the only way you can have clean cable routing with Gucci cockpit components like your THM stem. Also, does WyndyMilla even have a fork that supports fully integrated cockpits? All the models on their website show bikes with standard internal cable routing setups.

Check out their Insta from Bespoke 🤯🤯 although the bike is branded a spoon so I wonder if they are dropping the Wyndy brand?..
