Seems its not a widespread issue but just a personal issue to me.
It's that the clamping surface is relatively small with possibly sharp edges or something, causing the pressure onto the bars to be higher at a given torque on the bolts. Add vibrations and potholes that over time cause the bars to delaminate around the clamp area. Just use as much torque as you need and take note if you need to tighten the bolts after a while. It could be the beginning of delamination.
I have had several uno6/7 stems and I still use one today with a handlebar that isn't ultralight. With that it seems to hold up fine. Take care with ultralight bars however.

What's not worked:
J&L alloy bottle cage bolts - had some good sets but the last ones I bought were badly made and couldn't fit an allen key in the head. Had 1 snap on me too once.

Yep, exactly. Uno stem is fine. Also tried superlight seatpost clamps, always had problems and went back to heavy Thomson seat clamps ... security and peace of mind first

What works:
-Ti or alloy bolts in as many places as possible. Steel is heavy. Particularly on seat post bolts can be huge.
-K-Alloy Uno stem.
-Alloy brake and shifter housings, but only in straegic places like RD loop, frame into rear brake and front brake because they can creak.
-DT Swiss Ti skewers (not ultra weenie but safe).
-'welterweight' 70g butyl tubes, again not mega weenie, but strong enough to do the job and light enough.
-Carbon saddles like Arione 00.
-Bikes with rim brakes, as they are a cheap way to stay light - trying to weenie some disk brake bikes is a headache.
-SRAM Force 22 or RED groupsets, athlough not sure about SRAM brakes and GXP cranks. Maybe run DA brakes and cranks.
-Ti cassette lockring.
-Ti cleat bolts.
-Slamming stem.
