Seems to me this is somewhat akin to the Hollowgram in a non-BB30 frameset question. Regardless of custom spindle or not, a standard ISIS crank was designed in the days of 68 mm wide threaded bottom bracket shell, thus an ISIS crank spider is not going to have the proper offset for a wider bottom bracket. So like the hollowgram crank, a longer spindle is simply going to move the chainrings out beyond the required chainline, effectively moving the inner chainring where the out chainring would be on a BB86 crank.
