What's needed cranks that are BSA with a frame that's Italian threaded?

If the cranks are for English bb then all you need are Italian threaded cups.

Do you mean BSA30? Then any Italian threaded external 30mm cups. E.g. rotor, tune etc.

How much weight do they add? Should I just get cranks that'll fit without modification?

THM's site for the Clavicula crankset says:
fits any frame with standard 68 mm BSA shell. Available for BB30, BB30A, Pressfit30, Shimano Pressfit, Italian threading, BB386 EVO, BBright™-Pressfit
It sounds like you'd need their Italian threaded cups.

The only thing I found fitting 30mm cranks to an Italian BB is that the shell is 2mm longer and you need to look at the length of the crank. You need 70 +7+7+seals and a bit more with ITA shell and external bearings where the sun's are 68+7+7+seals and a bit more with English BB. I've not looked at how the THM's allow for the 2mm difference?
