I think the company is out of business. I have no idea who sells them if they are still for sale somewhere.
You have to take the straps off the HRM and it attaches right onto the top of the stem.
I'm making one myzelf now. Thursday one part will come in and then I will see if it will work what I designed.

The bike isn't the problem
I have this and has treated my edge 305 well for the last two years. The only downside is that its secured by two thick rubberbands which tends to make it swing either up or down when hitting big bumps...

The parts came in today, I made a picture of the 2 plates I gonna use. The plates only need to bend in a corner of 90 degrees. The carbon tube needs to be cut in the right lenght and comes between the 2 plates. The 2 plates will be assembled between the font plate of my stem and the stem.
Hope you will get the idea.
I've put it together with some tape and made some pics of it holding it on my bike.

It depends on how much plates I order. And for what kind of stem, the vertical distance of the bolts comes pretty close.
To get a good result you need to bend the plates when te frontcap of the stem is bolted to the plates. For the carbon tube between the plates you need to measure the width of the stem, I used a piece from my fork that had to be cut.

I just realized I have 2 bolt stems.

I zip tied my Garmin 705 computer mount to the top tube of my bike... right behind the stem... it's more aerodynamic there and protected from damage.... you just have to look down a little more....
