i don't know what else to call it, but i have seen them on a few different bikes, and am wondering their purpose and effect. i have seen them on different brands of rd's, so assume it is an aftermarket thing.

anyone know what i am talking about?

You probably mean a tool which is centered at the RD hanger before installing the RD. You install the rear wheel and by turning the tool around you can measure but also help to install a perfect parallel to the rear wheel and casette mounted hanger and afterwards a perfect alinged RD.
Or does some other kind of adapter exist ?

nope, it is something that is installed onto the rear der and used while riding. it looks like it allows a tight 90 degree bend for the cable while possibly reducing the friction normally caused by such a tight bend (at least that is my guess).

Another shot of a Jtek shiftmate on my Cervelo. Campy Chorus 10 speed shifters DA 7800 RD and Ultegra 12-27 9 speed cassette. Very smooth.
