Peter Sagan's Synapse. Custom geometry beyond a shadow of a doubt.
His Evo is too, but the Synapse is a better example as it's impossible to argue against. Just look at it!
It really depends on how the frame is constructed whether it's custom geometry is plausible or not. Not all frames are monocoque like the ones from the mold petepeterson posted. Ones like those are definitely not likely to be made custom since it would require an entirely new mold which is not cheap. Different layups are entirely possible no matter the manufacturing process and most big name pro frames are probably different than what you can buy.

Sagan's mold is the same as Bennati's old mold. Apparently a few other Cannondale riders used it when the SS first came out.

Didn't Bennati finish up with Cannondale before the Evo was released? It's possible that they rode the same geometry, but it wouldn't be the same mold.
The actual cost of producing a new mold isn't as prohibitively expensive as people might think.

Sagans is a 58tt with the head tube of the 54cm. Custom tube to tube.

I think people get hung up on what mold's used to cost before CNC machines and advancements in CAM software. Mold's are getting pretty darn cheap compared what they used to be when they were made on manual mills.
