So I'm looking at a pair of new shorts and have found a good deal on Castelli Velocissimo. I have an issue with sizing however. According to their size guide, the 'Small' size is huge around the waist, yet the 3XL is too short in the leg...

My measurements:
Waist: 76cm
Inseam: 88cm

Any recommendations as to size?

It upsets me a lot how cycle clothing manufacturers refuse to produce clothing for real cyclists.

Not sure what to say about your sizing issue. The 3 pairs of castelli bibs I have all ran small.
I normally wear a size small but medium fits in castelli

In regards to things running small, is this the usual "oh they're race fit so they're small", or are they genuinely small? Shorts should be tight fitting.

I usually go for medium and run large in Castelli
76cm waist

I wear a small in Castelli, they fit perfect for me. My waist is 76cm, inseam is 82.5cm. I would say that if you were between sizes on their chart it would probably be a good idea to size up.

On the running small, I would say the sizing chart is spot on for race fit. If you are used to a "club fit" go one size larger.

The issue is the size chart starts massive! The Small is 80cm waist! So going solely on that I'm screwed for race fit.

I guess a lot of it is down to thigh size - as two people with my waist size use very different sizes (Small to Large). My thighs are approximately 50cm (measured over my trousers at my office desk with a tape measure haha)

I believe since the size chart does not list a range, the number listed is the maximum that will fit. Shorts stretch to fit, but there is a maximum that they can stretch. My thighs are 55cm and I could not imagine wearing an XSmall.

That is very logical, hadn't thought of it that way, thanks!

As others have stated, I also sized up in Castelli. Shorts went from M to L and jersey from L to XL. I am 6 ft 1 in tall and 170 lbs. hope this helps

Older Castelli stuff fit pretty much like everything else (except gloves, which ran way too big)

Newer stuff runs small (and gloves still run big)

It seems like some of the lesser expensive shorts run a little bigger than upper end, but that may be due to compression fabs and what not.
