Best off going for a tight fit.

As I said, mine are a large and I wear them over size 46 Specialized S-Works XC shoes.

A tight fit is definitely what I am looking for. I have read other reviews that said when people sized up the cuff at the top was then too loose to seal around the lower part of the leg. I was afraid I might have the same issue with my skinny lower legs.

Alright everyone I received the rapha deep winter shoe covers in size medium and they are a perfect fit. It takes about a minute per shoe to put them on with no excess baggage anywhere. For people asking the question in the future I would recommend a size medium if you wear a 41.5 in sidi. I believe a 42 would also fit but would be slightly more difficult to put on.
