Dura Ace Rear Shifting Problem - Suggestion Please

Setup
Crank - DA 7900 SRM 53x39
Shifters - DA 9000 Brand New
RD - DA 9000 Brand New
FD - DA 9000 Brand New
Chain - DA/XTR 900 Brand New
Cassette - Ultegra 6800 11-28 Brand New
Problem
Rattling/jumping noise on 13t only, both 53/13 and 39/13 The noise will go away if I turn the RD adjusting barrel counter clockwise 3 clicks. (tightening cable tension) However that will make downshifting really slow - especially from 53/23 to 53/21 or 53/21 to 53/21 to 53/19. Shifting is perfect for all other gears.
Solutions Tried
New Ultegra chain - little quieter but still rattling.
New Ultegra cassette - no change
New Ultegra cable - no change. The rationale is teflon coating can cause cable to 'hang' Funny thing is the DA cable already shown a lot of wear and tear after less than 50 miles. (coating is already peeling)
All components are installed by a top LBS mechanic who has been working on my bike for over 15 years. He is going to call Shimano USA tomorrow to see if they know anything. But I also want to hear what people think in this forum.
My main questions are:
1. Is downshifting from 53/23 to 21 and 53/21 to 19 normally slow? I'm still recovering from achilles tendon surgery happened 3 months ago and that's why I'm riding 11-28. In the past I never have ridden anything bigger than 25 and that means 53/21 is most extreme gear I would have gone.
2. Is 53/39 good with 11/28? I believe Shimano said it's compatible but that doesn't mean it's good.

Couple of things...
Adjust your limit screw to prevent the derailleur from moving so far over while in the 11
Check your hanger alignment
Check for crossed cables if running internal routing
Adjust the b screw to put the upper pulley closer to your cogs if possible.
Check housing ends for any snags which could cause cable drag.

Had similar problems with an Ultegra cassette, turned out I'd accidentally put a ten speed spacer between the sprockets. Don't put two cassettes in the same solvent bath would be the lesson I learned.
Mind you, you're getting it across different cassettes and chains, so that rules that out. Something hung up or slightly misaligned with the shifter internals?

Thanks everyone for suggestions.
1. All cable route externally.
2. Hanger was checked and aligned.
3. Various positions were tried for B tension.
4. A brand new ultegra (no teflon coating) was installed. No frayed.
5. All cogs are installed correctly. I can see the tooth # facing outside.
6. RD jumps when chain goes forward and backward.
7. All spacers have 11s mark on them.

Have you checked:
1. If the RD cable is clamped to the correct side of the clamp screw?
2. If you have left a spacer for a 10-spd cassette on the freehub behind the largest cogs?
When the RD jumps with the chain going forwards and backwards, which cog(s) is the chain on?
