Mysterious pedal insert failure

There is nothing that was wrong. You may look for faulty things, but i can assure you, there was none.
Even FSA wanted to find "blame", but they could not.

12 Nm sounds low, I'd torque to 30-40 and check regularly in the beginning. Use light grease, a pedal washer and no loctite.

However, using grease on threads or Loctite, those two are really each others opposite!

I try to figure what to do on the new crankset. Should i use Loctite on the none drive side crank arm (38-41Nm) and possibly even pedals?


I would not use loctite on the crank mouting bolt, just torque it up correctly with grease, if you using 38nm you need grease, otherwise you will never get it off again.
With the pedal thread I would use loctite
