Yeah, our local fitter suggested E-soles for her high arch and high metatarsal.
She's been running some shims on her "bad" leg and it helped but still isn't perfect. Will remove and try insoles prior to cranks.

Riding with spouse is great, and yes make her comfortable. Get her a good bike fitting, it will make a difference in comfort and performance. Best money spent imho.

I'm happy to report she got a set of "E-soles" insoles for her cycling shoes and she has 2 rides (28mi/40mi) both w. ZERO knee pain!
We also had her test my friends Hi-mod Synapse 48cm w. custom 155mm cranks and after a 20min spin she really liked it and felt a smoother power delivery/stroke w. the shortened cranks. hmmm.. Might have to try a set of Cobb 155's and see how she does on all different terrain w. them.

Hell I ran a 5K with my 9 year old.. and my patella has been a world of hurt.. I torn up alphabet soup nearly all ligaments in my knee the other year and don't have much a meniscus left... just spent 35min+ at high cadence on the trainer and my knee feels worlds better. Bike is setup up with 30degrees of knee angle. I just don't see how a shorter crank is the fix ?

I was going to suggest Lightning cranks but they only go down to 160
