Selle Italia Carbon Monolink post - stripped nuts?

I'm already on my third Selle Italia seat post after only 6 weeks.

I've fitted each of them per the instructions using my torque wrench, but after fitting and riding them for a few days, when I've come to make a minor adjustment - as I UNSCREWED the fittings, the captive nuts have stripped! And then the fittings cannot be tightened again, seat post ruined.

I'm not using threadlocker or doing anything else weird.

Has anyone else seen this happen?

Obviously there is something wrong, I would start with the torque wrench, proper specs, then operator.
